Trunks ports wont release 4

At site, the local vendor had move lines 1 and 2 to line 3 and 4 about 6 months ago.
Line 1 and 2 had stopped working and there fix was to move to trunk ports 3 and 4. Worked.

Customer has now added a third line to the BCM50 and need port 1 fixed.

I installed the new line to port one and it work right away for a bit.

Shortly afterward I found that the line would go hook and would never release.
Replaced amphenol and moved exiting working line 3 back to trunk 1 and had the same issue.
A system reset done and trunks wont release on ports 1 or 2?

Any suggesstions?
Replace BCM to fix ports 1 and 2.?

-Make sure all lines in programming have Disconnect Supervision set to Yes
-Test
-if still an issue make sure the carrier also has their end programmed, sometimes called COD Cut Off on Disconnect

To test the port move a working line to it to test.

I did move the working line on trunk 3(customers line 1) to trunk port 1. Same problem and trunk wouldnt disconnect.
Local vendor found issues on line 1 and 2 a while back and moved to port 3 and 4.
Im not sure what happended, possible power surge?

All lines are programmed the same with disconnect supervision- yes.

Local vendor has told the customer that the BCM50 needs to be replaced to get trunk ports 1 and 2 to work?

Any other suggestions?

The BCM50 is at Release 5. Would this patch still apply?
