I have a compact Definity system, running G3V6i.02.0.120.5 which I'm trying to clear out configurations on from it's previous life as a small hotel PBX. There are two problems I've encountered while trying to reclaim the dial plan and generally clean up the switch. I'm not sure if they are both related to each other, but I'm stumped and need some guidance.
The first problem is that there are 2 station circuit packs that are showing in "list circuit-packs" that are not physically in the cabinet, but I still can't remove using the change circuit-packs screen. I am still getting the "" " Must remove translation on the board before changing circuit pack type" error. I looked up other related threads on here regarding this problem, but the solutions listed didn't help me. I was able to remove all stations except one (see next problem below), so I don't think there are any stations holding up the removal of the boards. And it seems that none of the ports are stuck or otherwise held. Here is what I am getting on the list configuration carrier 01a screen for both cards:

The next problem is I seem to have a phantom extension that I cant remove or find what object it's assigned to. It's the only item listed in list extension-type. Of course when I try to remove it, I'm met with the error "Extension exists but assigned to a different object". I tried checking the console parameters, and its not used as shared extension. And there are no consoles configured in the system either. There are no ACD items on this system, and of course there was no telecommuting use for this. There is no other clues that I can see that shed some light on where this extension is being used. Here is the list extension-type screen:
