Transposed Data on SO

Has anyone run into this:

You create a SO, and save it, print it. Then when you go back into it, it suddenly has additional lines from another, un-related SO.

It is cross linked. Go into the Utilities in the Library Master and run the ReLink utility.

I do that all the time and it does fix it, but what is causing it? How do I get it to stop?

This will happen when you have a lot of people in the same files doing a lot of data entry.
