Transferring Problem on inbound calls

Hi There, two clients of ours has transferring problems on their IP Office. when the switchbaord operator is busy on an incoming call, transfer the call to a user, at the same time another inbound call is ringing. when she transfer the first call to take the second call and tries to transfer the second call as well, the two inbound calls get connected with each other. like a conference. they are using a 1616 phone and the IP Office is on version 8.1 (56)
she does the following: answer the first call, (now another call is ringing) press the Transfer key, dial the extension number, hangup and answer the second call and transfer it aswell. this problem does not happen all the time, but most of the time.
any help?

user error , run a monitor trace on her extn and check the behaviour.

Check Busy on held tab to see if this is on.

Also enable Reserved last CA to reserve a call appearance to transfer calls.
