Here is my problem: I have an extension (for example) 3223, this person wanted his extension monitored for: busy, incoming call, and finally incoming call when his primary line is busy.

I got all those things to work, but had to be on separated buttons. The problem that I am having is that when a call comes in on the button that is set for "incoming calls when busy" the person who is monitoring extension 3223 can answer it and put it on hold. But cannot forward the call to anyone. We press the transfer button, but get no dial tone, or beeps. Is there anyone who knows how to get this to work so that calls can be transfered also?

I believe that you have to have two appearances of the
brdg-appr one for the first call appearance on the original phone ( button 1) and one to be able to transfer (button 2). I experienced this same problem and the only way I found to resolve it was to add another appearance
hope this helps

While on the call, press transfer, select a idle line appearance, key in the destination, and press transfer again. Hopefully that should do it.

Thank you both for the prompt reply, I will try them out and let you know.
