transfer and calling extention issues

I have a new issue at a customer. When they call any extension the speaker comes on instead of ringing. It also does the same thing when they transfer calls to any extension. When they transfer calls the speaker comes on so the call will not go to voice mail.

Answer could be in their short codes tab - are their dialing codes uniform with other users that aren't experiencing this issue, or do they differ?
 
This is happening to all phones all the time. I don't see any unusual short codes on any of the extensions
 
This customer uses auto attendant and when you enter someones extension through it the phones ring like they should
 
It really sounds like an issue with dialing short codes on a system level. Go take a look at the dialing codes, and look for something like this:

In that instance, I set up a test that dials a 4 digit extension, using the paging feature to auto-intercom the dialed extension.

If that's not it, what release and model of phones are you working off of?
 
Try this:

You can select to have internal calls automatically connected if you are not already on a call. The call will be connected, after the phone gives a brief tone, using either the phone's speaker or the headset as set by the phone's audio path setting.

1.Press Features.

2.Use the up and down cursor keys to highlight Call Settings. Press Select.

3.Use the up and down cursor keys to highlight Internal Auto-Answer.

4.Press the Change soft key to change the setting.
