Toshiba DK 40 card slot for RCOU

Not all that familiar with Toshiba and have checked what info I can find. I am adding CO lines 9--12 into the expansion cabinet of a DK40. I had tried a RCOU3 and that didn't work. Now I am trying and RCOU1 card, V.1. The led lights on the card light when I press a CO line button, I know the lines going in are good, the buttons are programmed to be the correct lines, but I can hear no dial tone. I did program to tell the slot it was a CO card, I beleive that is code 11. What I am not sure is that I have the correct slot number. Can someone clarify which program I need to program the card slot and which card slot this RCOU card is. It is above the other RCOU card.

Slot 15 in Program 03 is the lower slot to the back of the expansion cabinet. And yes the code for a 4 port co board is 11.
 
If your adding lines 9-12 you should probably use a RCOU/RCOS 8 trunks in one slot and use code 17 in program 03.
