TMDI Card D Channel error

I have a PBX on RLS. 24.24 and the switch went down and everything came back except for the T1. I force download the TMDI card, enable D channel, I get this error:
DCH:001 RECV FAIL (MTC FAIL 39) AUDIT
Also I have tried to enable TMDI, enable Loop, then the D channel and get same error.
I have never seen this and wondering if anyone has any thoughts or suggestions. I have swapped out TMDI cards and moved to different card slot.
 
Bypass the CSU, assuming one exists. See if that changes anything?

disable the DCH, Loop, then TMDI in that order, unseat the card, then reseat. Redownload TMDI, enable the DCH/LOOP. If it doesn't work after a re-seat, you most likely have a card that went bad.
 
It is working now. What we did was out the DCH and rebuilt IFC=ESS5, which was NI2. Re-enabled everything, TMDI, DCH, and Loop and the alarm cleared.
Then we rebuilt the DCH as NI2 and re-enabled everything and it came up clean.
Don't know what could of caused this, but it is working.
