tip and ring problem - help please

I have a phone that seems to have a problem. If you grab an outside line and dial a phone number, it will call properly. The person on the other end cannot hear you and you cannot hear them. Also, you cannot hear the dialtone, although it seemes to be there because you can call out. The simple solution seems like it would be to replace the hand set. I did that. Nothing. Then I replaced the whole phone. Nothing. I rebooted the whole phone system. Nothing. I have made no changes to the phone system programing and the phone worked fine at the beginning of the week!
It was suggested to me that I have a tip and ring problem. I connected a phone directly to the system port and it works fine. Do I have to run all new wire to the office where the phone is normally kept?
 
I'd say terminate the cable again on the jack.

Don't know what tools you have, but you could also use a cable tester to test continuity on both sides.

A toner would work too, try sending one on the center pair and see if you can short it on the other end to stop the tone. If so... you've got continuity in the center pair, and it's probably just a termination problem.

Very low chance of the wire actually being bad unless some work was done recently around it and someone hit it with a tool or something.
