Telephone Survey After Call

Hello

We have a UCP100 installed with 14 SIP Trunks and around 30ish LIP handsets. The customer wishes to offer a telephone survey at the end of every call. A simple question that will ask the caller to rate the service of their call in to the business. Does anyone know of a company/product or service that can do this? We need to be able to have a simple monthly report from the survey call too.

This is something I think you can do with Braxtel that the UK E-LG resell now does?
 
Simple way is to transfer the customer to an auto attendant to select 1 for yes 2 for no , if yes is selected they hit a mailbox with a prompt and they can leave feedback .... its obviously a really basic offering , they would have to run a manual mailbox report for stats....but it would work.
