TAPE DRIVE PROBLEM

I have a 1/4 inch tape drive which will rewind, erase, write and read small files.
However, if I try and write larger files(although not nearly large enough to exceed the tape/drive capacity), I receive and I/O Write error code 5.

Is this a fault on the hardware or is there a problem with the device file?
 
would suspect it is a hardware problem. But could be configuration error.
What kind of tape drive? How is is configed? What version OS?

Not sure about the config or type. Its on a system I've inherited from a 3rd party - the drive worked fine until a week ago when this error started.
Sco Version 5.0.5
The drive is written/read using device /dev/rStp1
