Table of Contents Applies Random Styles, etc.

In the TOC of my book, I have two serious problems (and 1 minor issue):

a) Some of my lesser headings appear in the table BEFORE my major headings -- no matter where they are in the document;

AND

b) My TOC keeps assigning random paragraph styles to my entries. When I fix them and reapply the style, they revert.

My minor issue is this:

Some of my entries in the TOC end up with a peach-colored background. It goes away if I hide hidden characters. Each time I recreate the TOC, it makes different entries peach. I have no idea how to fix this.

I tried the buggy document fixes recommended in the sticky on the Adobe Forums... but they didn't fix my problems. I have the most recent updates, and am using InDesign CS2.

Any thoughts?

OK, well I got the "applies random styles" bit taken care of... apparently InDesign doesn't like "small caps" in paragraph styles on a table of contents. As soon as I remove that setting, the problem disappears... but if I set it back, everything gets all messed up. BIZARRE. But this is here for others, in case they have the same problems. I'm still working on my other 2 issues from above...
 
Hmmm... so now I've realized that the way the table of contents has been arranged isn't in any order whatsoever... lesser headings and major headings are all over the place, in no particular manner. Any thoughts on this?
 
K.... fixed the last weird issue, too... It seems that I when I had placed the "Heading 1" text on a hidden layer (so it wouldn't be seen -- I already have the Chapter title on the Master page and don't need it twice), I had also placed the text frame outside of the margins where the rest of the text was. Apparently, this threw it off. Once I moved the hidden Heading 1 text into the margins, the order was fixed. YAY!!
