Thanks very much for all who helped. It appears that the CF card writer was defective.

All the partitions etc are set up and I think its soooo close to getting to the maint screen but this error comes up:

gzip: main.run.gz: not in gzip format

Ok its running!!!!! I'm going to post the various attempts in the hopes that this maybe of some help to someone else in this situation.

1rst:

Flashcard Brand: Transcend 8GB
Formatted Capacity: 2 GB
File System: FAT32
OS Version: 4.0.4.9

Results: Unable to format internal HD, could not proceed

2nd:

Flashcard Brand: Lexar
Formatted Capacity: 512 MBs
File System: FAT32
OS Version: 4.0.4.9

Results: Installation took but this error happened:
Adaptation Layer Coordinator FAILED to start.
System would reboot, unable to continue.

3rd:

Flashcard Brand: Lexar
Formatted Capacity: 256 MBs
File System: FAT
OS Version: 4.0.4.9

Results: Complete install, successful system update from V4.0.1.11. Able to get to maint screen. Able to enter MOC, system identity code and mitel options password. Able to change system IP. Rebooted, able to get back to maint screen, able to restore backup. Success! system is running.

I really appreciate everyones help, A simple upgrade turned into a total nightmare. Ultimately RossBC had it right (
