May you please help on how to configure LCR using the information as shown below.

Sorry, I am just packing to go on holiday, so I can't help. However, you have 02 in multiple service options. so you need to indicate the priority for each service. For example ISDN first, PSTN second and Voip third.
 
After creating your tables in 8A4XXX, verify your AMND or CMD 855 satisfies your dialing parameters.
Looks pretty straight forward and well organized.
