Submitting multiple records simultaneously

I have a form that I would like to put online to collect data about physicians in our hospital. The forms results will go to an Access 2000 database. I would like the user to be able to complete the form with data for 5 physicians on one form. Essentially this means that 5 records would be submitted to the database at one time. Is this possible or do I need to design the form to submit one record at a time?

(I would be using Front Page 2002 to build the form)
