structured menus

I create menus on the fly but the list of options is getting too long. How can I add structure to the menu either by grouping by each letter of the alphabet or perhaps a group of letters where there are too few entries.

I would break the application down into discreet areas of activity, so that you can then group the sub-options in a logical way.
For example, The application may have File Maintenance,
Customer Care, Reporting, etc
Within each of the Main options, group the Options that best fit within the application Area.
File Maintenance
Maintain Post Codes
Maintain Service Plans
Maintain X......
Customer Care
Create Customer Account
Log a Call etc

Having created the logical heirarchy, you can then use this to build the menu/tree view based on the users rights to the individual options, suppressing those parts of the application that they are not authorised to see.

Sorry I can't be more specific, but obviously every application is different, and it is is difficult to get an answer that fits all! However, hope this is of assistance
