When using the rectangle tool, holding down the shift key supposedly produces a square. Likewise when holding down the shift key while using the pen tool supposedly forces a horizontal, vertical, or 45 degree line. However when I placed a 45 degree line over one corner of a square produced using the rectangle tool, the line did not intersect with the opposite corner as it would if the rectangle were a true square. I am not sure whether it is the rectangle tool that is not producing a true square or the pen tool that is not producing a true 45 degree line. Can anyone tell me why this is occurring and how to correct it?
 
Check your General preferences:

In the General preferences, Check the value entered for Constrain Angle. It should be 0.
