Re handling spot colour in Photo Shop:
Should files be cmyk? then add spot channels?
Exporting to pagemaker v6.5É
The manual is not too clear on the above, when I open an exported dcs2 in Acrobat distiller and Reader the resulting file is blank, hmmmÉ

I have used spot colour in Adobe Photoshop 5.5 without any problem.
I used it with a CMYK image. Then added two spot channel(s) through the channel pop-up menu.
The file was saved in DCS 2.0 eps format. Imported in Pagemaker 6.6.
Made the Postscript through PM6.5. (Separated). Distilled and opened in Acrobat Reader. I got all the 6 colours on separated Acrobat pages.

Try this instead of distilling the DCS directly.
(May be on Reader the file appears blank. But on the final imagesetter output it my come OK.)
