Speaker Phone Monitoring?

Is it possible to listen across the speacker phone mics in a room without being noticed? If so how is this achived? How is it blocked from being achived?

Another quick question if you have time to answer. How do you monitor calls?

example ext ### gets a call or is on a call I want to monitor it for quality assurance how do I do this?
