SPA8000 VOIP with BCM50

Recently we switched from our ISP VOIP provider to using our own SPA8000 and VoIPO. We are having issues when the lines are forwarded out from BCM where the line will get stuck and not hang up. I look in the BCM monitor and I'll have a ? on the line status. Does anyone have an idea on what would cause the BCM to do this?
 
This also happened when someone was placed on hold if that helps.
 
not sure on the bcm but on the norstar the trunk mode unspr or super may be the cause
