Installed on a CS1000 release 4.5. I had a Polycom SoundStation Analog Conference phone not dialing the correct number of digits. It would change and complete the dialing string before I was finished dialing. Changed physical path to phone, removed, changed and rebuilt the TN. I discovered that the phone would only misbehave when installed in one particular office, in all other locations the unit worked just fine. The source of the fault was - the echo in the room. Enough echo was present to cause the phone to inject multiple dialed digits via the microphone inputs. The solution was to mute the microphones while dialing. Not a pretty solution but is effective.
