Software "Issues" Database

I need to get a database to help us keep track of "issues" found in our software (It doesn't have bugs. Bugs are bad. However, sometimes it has issues, which aren't necessarily good or bad.), but I have no budget for this. Does anybody know of any free, or very inexpensive products to do this? I can build a database using Access, but that will add to my workload, so I would prefer not to. However, if the cost of getting a system is too great, that is what management will ask for.
