It may not be your Exchange server at all ... You may have an open mail relay from an IIS server that you have exposed on the Internet ...

Microsoft IIS provides three ways to secure mail relays:
a) Client authentication
b) Relay restrictions
c) Connection control

Client authentication requires a host to log in before using the mail relay. Relay restrictions limit which domains can send mail, while connection control limits
which machines can send mail. The method for securing the mail relay will de-pend on the application server and its business process.

Connection control is probably the least intrusive method to secure an open relay on an IIS box ...

1) Launch IIS Management GUI
2) Right click on the SMTP service (if listed) and bring up properties window;
3) Select the Access tab, and click the “Connection…” button.
4) Enter the IP addresses allowed to use the SMTP server, or the sub-nets allowed to use the SMTP server.
