SMTP queue - Remote Delivery

My queue is showing several domains with 'Remote Delivery' in parentheticals. Does this mean inbound or outbound emails? 99% of the time, these domains are spammers so I'm leary of approving the email delivery because I'm not sure who is the recipient: us or them?
 
If you look at my post above i had a similar problem. The first thing i did was to use network monitor and detect were the spam is coming from. It could be several things. It could be a virus on the network, you server could be configured as a relay server, or someone is vpn'ing into the server. I would first start by running network monitor, turn on your smtp logs. Scan the network for viruses, change everyones user account to require them to change there password on next log on. lock down users who are able to vpn in. I noticed on my 2 situations that after a period of time it dies down....then restarts..but after i changed everyones passwords and did the above both servers are ok. also your server will bog down real bad and the space on your hd will go down to 2 MB..mine used up around 3GB of msges in ques.
