Slow Printing on VPN

I have a 100+mbps server that i'm hosting a database for a client. The client has software to connect to the database and they are able to download quickly and use there client for the database at good speed as well. The only problem is when the client tries to print it to there printer it's taking 3 mins per page. When there database was being hosted on site there was no delay. Is this in the norm? Does there upload speed have much to do with printing?

MediSoft is the application there using

Windows 2003
1GB, 2.6 Pentium
OC12

Client
WinXP
Pent 4
Cable 3mbps/384kbps

What about printing via office or just generic printing.
Perhaps Medisoft has a problem spooling via a vpn connection ?
