SIP peering or Registration

never setup peering with the IP Office as far as I remember
either registration or the provider white lists the public IP
caller ID is easy to set depending on what release you have the setup is slightly different but flexible. In the US however you can only use the numbers you own on that trunk while some other countries are not as strict or not care at all and you can send whatever you like
