Single PN IPSI going out of service daily

I have an s8500, CM2.2, single PN/single IPSI that goes out of service daily and then reconnects immediately. It is a very short out of service condition before it reconnects back to the network and starts talking to the s8500 again. I've run error logs and below is what I pulled from today.
We've recently installed new TN2312BP HW3/FW40. We've also moved IPSI, CLAN, Medpro, s8500 ports to different network switch. Any ideas on what may cause the IPSI to drop? Thanks for any assistance.

May seem like a silly question but humour me. Please post a list config of the ca that is having issues. "List config car 1a" and the serial number of the 655 power supply

I did a list config carrier 1a, and it doesn't show the 655 power supplies in the config. I did a ch cab 1 and it shows a single carrier stack, I copied it below.

Did you replaced the IPSI board?
Which hardware and software vintage does it have?
How much analogue boards do you have in the system?
 
We sure did, new IPSI is TN2312BP, HW3/FW40. Old IPSI had same problem, and it was a FW21. There's a total of 9 793B analog boards.
