Showing date parameters on report

I have modified an existing app that has a report requester form. I have created a new report and call it from a button on the requester form. This form has a date range that I want to print as a heading on my new report.
On my report I have 2 text fields and I have set their control source as follows:
=[Forms]![frmReportsRequester]![ApptDateFrom] and
=[Forms]![frmReportsRequester]![ApptDateTo.]
When the report is run the fields show as #Name?

Can anybody please help?
 
Assuming you have spelled the names correctly is the frmReportsRequester form still open?

Thanks RG. Yes the form is still open. The odd thing is if I open the database in design view and run it the date works OK. If i open it without holding down the shift key it doesn't.

Any ideas?
