1.We have 3 operating systems with same configuration of servers and applications, but on one operating system the application works well but on the other it does not, what could be the issue? O.S is same on three.

2.Scenario: There is one proxy server and one Single Sign On server, when users login to the application it does not reach the application server files and Welcome page occurs again and again, what might be the issue?

First of all, the error you're getting. The "doesn't work" doesn't offer much info.

Filesystem implied? Native libraries? Same version? Exactly same configuration? Same drivers? Databases? ...
