PLEASE CAN SOME ONE HELP ME TO IDENTIFIY THE CORECT DRIVER FOR THIS MODEM I REFORMATED MY SYSTEM AND CANT FIND THE DRIVER HERE IS THE INFO

THIS MODEM HAS 2 BLACK CHIPS ON BOARD
I CAN SUPLY THESE NUMBERS ON THE CHIPS
IF THAT WOULD HELP ANY MORE ANY HELP
IS APRECHATED TA IN ADVANCE

hi thanks for helping i see the driver i nead but when i select it it goes to the midle of the page would it be posibale to put the link here for the actual file i nead so that way i no i have the corect one thanks mike
