Report Documentation - Fields and Aliases

We are implementing Salesforce and we are moving our Crystal reports to the SF reporting tool. The implementation partner is requesting that we document the existing report by giving them the column header name and the database field name. Something like:

Column Header Name / Database Field Name
Descp / MSPMP100.DESCP
Part / INITSUPPV.IPART

Looking for suggestions for an application that will do something along these lines.
