we have a combo card that has 3 damaged CO ports on the ATM, can we replace the ATM4 on a combo card?

Yes, it will be fine, you can even swap from ATM4 to BRI4 and it works (the card, not the analogue lines) :)

If you have 3 damaged atm ports then why not replace the whole combo card as you may have defective digital or analog ports as well, replace just to be safe

I (nearly) agree with Joe
What caused the CO ports to fail?

if it is suspected that they have been destroyed by external inputs (storm damage etc.) then I would consider the WHOLE system to be suspect
it wny be working now but it could simply be a matter of time before something else fails.

Do things on the cheap & it will cost you dear

I agree, replaced the whole card to avoid deeper problems

FYI, Avaya does not support replacing the ATM only

thanks for the comments
