Reject Publishing request with email

I am looking for a web part or code to change that will allow for comments as to why a document was being rejected and send a email to the individuals are selected to approve the document

to some code where you can modify the reject cycle. Basically it's a custom page so when a reject happens on a document, it takes you to the page and you can enter your comments as to why it was rejected. The reject comment is then emailed to the original sender.

It shouldn't take much to query the document to get the list of approvers and email them as well, if that's what you want to do.

Im trying to do the next thing.
When ppl post something is a personal form (like asking for days of) it has to be aporved and if approved it should be copied in apublic form.

How can i do this??
