Redial or dial from UC causes busy tone

We have had an issue arise all of a sudden where users hit redial or dial from Unified Communicator when they click a number in our CRM app, they get busy tone and their set displays BUSY. If they dial the same number manually it is always OK. This used to work fine, and I'm not aware of any changes made to the system, but I would put it down to COS. Any idea's.

This is an MXe MCD 4.1 SP2 and the sets are 5340's and UC is Advanced 3.2.
 
Check the dialing prefix on the UC server. It should contain a 9. That puts 9 ahead of any 7 or 11 digit call. Without the 9 you will get reorder tone.
 
Thanks for the reply SXWizard, I've checked our UC and the 9 is in. It was already there and has been wroking before, plus that wouldn't expain why REDIAL does the same.
 
Did you change the computer "Phone and Modem" dialing rule under Control Panel? It would cause the issue as well if you program prefix 9 as well prefix 9 in UC setting.
