Random Warm Starts

I have a small IPO R11 that has randomly rebooted twice in the last two weeks. In each case, audit trail says it was a warm start. Today I had been doing some programming. When I was done I made 2 test calls with the customer and the system rebooted while I was on the phone. In other words, I saw System Status go down and I definitely did not tell the IPO to reboot. This is happening spontaneously.

I think this is a bad control unit. Any other ideas? Could a bad combo card or faulty SD card cause this??

Full disclosure, in June the system was randomly doing cold restarts. We installed a UPS and that stopped. All was good for over a month and now we have the warm start issue.
 
Overheating PSU, have had this before and replaced CCU psu and all good since
