I am running an Adaptec 2410 with 3 Seagate 120GB SATA HDDs (RAID 5). Problem is every now and then the RAID controller will sense one of these HDD to be offline and sound the alarm. But when I restart the server and check in the RAID controller's BIOS, the faulty HDD will become "online" again and the controller automatically doing a rebuild on the RAID.

At one time, while one HDD was reported offline within the Adaptec storage manager software. A rescan on the drives will get the offline HDD back online?

Running out of ideas, please help.

Re-seat all connections.

Run diagnostics on the discs. Either the manufacturers, or something like spinrite. You will need to connect the drives to an ordinary IDE channel to do this.

I would suspect either the controler or the disc(s) are developing a fault. (If a re-seat doesn't fix it)

Thanks for the reply stduc,

I've changed the cables at least 3 times already.....
Have also ran Seagate's HDD check and all HDD checked to be fine.....

You seem to have eliminated the drives as the problem. That leads me to suspect the Adaptec 2410.

However, before the controllor gets the blame, I would verify that the power supply is more than adequate and I would check that the drives aren't running hot when under load.

2x 80mm intake fan are installed in front of the drives.
The PSU is a good 550W, I'll try to stress test the system with a new PSU tonight.
