Radiant RJ-45 to RJ-11 adapter

I am out of the factory Radiant RJ-45 to RJ-11 adapters for com ports 3-6. Does anyone have the sheet or the figure for making one. Thanks.
 
If you're talking about the cables needed to connect a receipt printer, I have a cable here that we use.

For clarification, the following pinouts are viewed with the cable in my hand, with the contacts facing the ceiling (up) and also facing myself. The plastic clip is facing away.

The RJ45 end looks to be using the standard T658B wiring schematic. Reading from left to right, the wires on the RJ45 end are:

Orange/White
Orange
Green/White
Blue
Blue/White
Green
Brown/White
Brown

With that same orientation (contacts facing up and towards me, clip facing away), here are the RJ11 contacts.

Orange/White
Blue
Green/White
Orange
Blue/White
Brown
