qmail bottlenecks

currently, my qmail (netqmail 1.05 installation on netbsd) tops out at around 35 simultaneous remote deliveries, even though:

- big-todo patch applied
- dns caching enabled
- plenty of cpu/memory resources available
- no softlimit set on qmail-send
- full t1 connection
- concurrencyremote set to 120, concurrencylocal set to 20
- it's essentially a dedicated mailer for our newsletter

i'm trying to figure out what the bottleneck is, any advice?
 
Q: How are you measuring concurrent outgoing?
Q: Have you restarted the qmail-send service since setting the concurrencyremote value?
Q: Are you scanning your outbound messages on this box before sending?
Q: Does your qmail-send run script honor the value from concurrencyremote file? (please post if unsure)

- checking /var/log/qmail/current, I haven't ever seen > 35/120
