Punch down layout for BRI LINE TN2198

Thanks in advanced. Can anyone tell me where I can find the punch down layout to connect BRI LINE TN 2198 to a 66 block? I am trying to find which pairs to punch down to to connect to the ports that I have assigned to the ISDN lines.

The first 12 pairs in the cable are circuits 01 - 12

With multipoint, you can program and connect 2 different stations to the same port for 24 stations on one card.

Thanks. It looks like I punched it down right. I have error BRI PORT 1 on all 3 ports that I set up. I also have BRI DAT error 3329. I think BRI PORT 1 is the test to see if anything is hooked up to the line, not sure what the other error is. Any suggestions.
 
Its been a while since i punched one of these down. But if I remember right on a regular USOC RJ-45 jack you punch the first pair down on the white blue and the second pair on the white green. If you are trying to connect this on a Cat 5 jack the pin outs will be a little different. Look at the wiring scheme on the back of the jack to get the right pinout.

PRI is blues/Orange. RJ45 1,2,4,5.
BRI is orange/greens RJ 45 1,2,3,6.
Can you put up a screen snap of your BRI-DAT station form?
