psd file why is only toplayer printed

InDesign printing problem.
A PSD file is placed in InDesign, the file contains 3 layers, from the bottom a layer 0 (not a background layer) with a citymap on it, layer 1 wich contains a stroke, to indicate a certain area of the map, and a layer 2 with another stroke. All layers have the status Normal in layerpalette, the two toplayers are transparent except where the strokes are. The file is placed in InDesign, and then printed, but only the toplayer from psdfile i printed, this is in InDesign CS version, no layers are turned off anywhere, why?

I'm just throwing this out there not quite sure if it's right, but should one of the layers be called background at the bottom. I know when you create a new psd file - the default layer is called background and then when I add other layers the background layer is always at the bottom with my working layer above it.

Does your file have a background layer?

None of the the layers are backgroundlayers, and its printed in a professional print house with a postscriptprinter
 
In your .psd file make sure you do not have extra channels besides CMYK, Cyan, Magenta, Yellow, and Black. Sometimes when you work with paths or shapes in PS a new Channel is created and acts like a mask blocking out items.

Also, does it need to be in .psd form? I find that having transparencies in InDesign requires flattening before being sent to print. Your Layers have that transparency and if you are not flattening your InDesign file when you send it to the printer - you can also encounter those file issues you're having.

If you do not need the file to be in .psd form I highly recommend .eps.
