PRT a NET card?

Had a power surge and took out our NET cards. Need to know what LD and CMD is to PRT the NET card to find out which NET card is going to which LOOP and SHELF. I have tried most resources and no such luck. Thanks
 
If you know what Network card is down, that is the Loop number. As posted, the Loop is determined by what slot it is physically located in.
