Programming Lines on Norstar MICS

Trying to program lines on Norstar DR5 MICS, brought system up as default square. Lines 1 & 2 appear, trying to program an additional line right below the other ones. This line is default programmed as Call Forwarding. How do I remove that button & create line 3 & so on M7310 phone.

PS. I have already went into programming line config & set to appear & ring on this specific extension.. Still no luck, it continues to revert back to the default square system??
 
so you go to set ..do f * 0 and set says lets say ext 21 or 221 ..then you went into progamming and assigned line 3 to appear/ring on ext 21 or 221 and when you go to set 21 or 221 it still says "when you do f * 0 forward...then you go into programming again and to set 21 ot 221 does it say line 3 is still assigned to appear/ring or did it go back to just lines 1 and 2 "no 3" if so you have some bad software

This is a DR5 8x24 system not MICS as I previously posted.
I did perform a F*0 & the button will not change to Line 3. I have 2 ls/ds cards in the main cab, I did not add the fiber Mod yet. But with the 2 cards installed I should have the ability to add 2 more lines right? Lines 3 & 4 right under 1 & 2. I was able to remove the call forward button that was previously on button #3, but still no luck when trying to add additional lines. It's still showing in programming, however it's not working still??

Yes I'm plugged into the first pair on the block, I verified I am at x221.. Thx
 
You say it is a DR-5 8X24?? How in the world were you able to get the trunk cards to fit into the cabinet??
