problems over voip line

I have a customer with 2 different sites one a IP406 3.1(56) and the second an IP401 NG 2.1(27) which I believe is the larger of the two smoffs. They've had their VOIP line set up now for a while working fine but all of a sudden they have started experiencing one way transmission. If they call between sites the caller where the smoff is can hear clearly however the site with the 406 can all hear slight muffles and the occasional voice, any ideas?
 
The site that don't hear any voice could have a unit on the network with the same IP-adress as the IPO.

It could also be a routing problem, heavy traffic on site network, or bad quality on the VoIP line.

Or something else :)
 
I can run a scan on there network to check the ip address issue though I'm pretty confident it wont be this as surely other problems would be occuring, with regard to the heavy traffic it's only a line used for voice and the call volume isn't much at all, the network issue must be something through the IP office settings as these are direct connections!?
