Poor duplexing on SIP calls on IPO 406v2 - quiet calls

When a user has any noise around then they can hardly hear the person at the other end of the call when on a SIP call. Audio level seems ok to party on PSTN. Even with little background noise users are still complaining that they can hardly hear the person they are calling. All users are on 9335 POTS (Phone 30) on v5.0(8), ITSP=Gamma. This is causing me massive problems and I dont know where to start. The system is on a public IP address (LAN2) on a 2Mb (synchronous) line. ISP Griffin (direct interconnect to ITSP).
 
For sh*ts and giggles I'd try another provider just to get a baseline.
 
If the ISP is Griffin, I would give Gradwell a try for your sip trunks.
we have a non-internet private MPLS SIP product with SALa on packet loss, jitter etc., but guess I cannot advertise here....
