Policies NOT Executing?!!

We've recently gotten complaints from users that their backups haven't been running through the scheduler. We've submitted some bpsched logs to Veritas Support. Have any of you encountered issues where Policies are not being executed? If so, what was your resolution?

It all depends on the type of policies? Are they frequency based or calendar based? If frequency based - What are the specifics in terms of backup window, frequency and retention?

Frequency-based Policy
Oracle backup
Window schedule is set to kickoff from 10pm to 4am
Policy kicks off once every 12hours with a Retention level of 3 weeks
Multiplexing is set to 4.
