PHP FORM MYSQL

Does anyone know of a good information resource that I can find examples of PHP forms to add entries in MySQL databases.

You can try to read a book called "Professional PHP Programming" published by WROX. (this book is red in cover and the cover contains five black and white photo).

In my opinion, this is an excellent book for reference.
However, i still think

provide the most up to day information.
