phone volume problem

I have a VoIP system in place running over a shdsl line. there are about 9 users with about 4to5 users on the phone at the same time.
Once in a while when on the phone the volume get really low to the point that I can hardly hear the other person (not on a cell phone)

Any Ideas what could be wrong?
 
Is it certain calls that the problem occurs, or can it happen *during* a call (or go away during a call)?

Does it happen only calling specific people?

Does it happen only calling from a certain phone?

Does it matter if the calls was inbound or outbound?

once in a while any call just not one person. I was told that I might be loosing packets from my ISP. Is that true?
 
I belive your ISP are taking out of their backsides (I may be proved wrong here).
Remember this is Digital info not analogue. Packet loss results in broken or repeated speech.
I haven't really heard of this on IP, but a couple things to look at>

Mic levels on phones (is there an "Auto" option) If so try setting to fixed.
Turn off silence suppression, I've seen this do odd things before.
CODECS what one are you using? You may want to try one with a lower or even higher bandwidth.
