Phone rings busy after 3 Lines Tied up

I have a phone with 4 appearances of the same extension. Is there any reason why the number rings busy after 3 of them are occupied?

There is an option to reserve one call appearance for conferencing and transfers. If they are all tied up you cannot do either. Its "restrict last appearance" on the station.

Does the station have a coverage path?

By default the last call appearance on a phone is restricted from receiving an incoming call so that your users will have a place to make conference or transfer callers.

And I would assume that if this station doesn't have a coverage path, and all incoming allowed call appearances are full, you'll get busy....
