I have a dead Phone 30 V1 module that is a couple of months out of warranty - Avaya says they won't repair. Anyone have any other US-based repair recommendations? Thanks!

I know of a US based repair facility; however, I work for said company so I do not want to engage in blatant promotion / selling (against ToS). If you are interested, post your email and I can email you the details.
Unfortunately, it is against forum rules to post email address... You can try my user name at that big LEC that serves most of the northeast US (name begins with V - they're a big cellular carrier also) dot net.
It's equally against the rules to make it so blatently obvious that anyone can figure it out I just post it like "myname at mycompany dot com" and it's ok.

Well, when I said "gotcha" what I actually meant was "that was way too obscure for my addled brain to figure out so I will not be able to help you" and, therefore, no email was issued, no contact information revealed. We can all sleep well tonight knowing that absolutely ZERO rules were broken (as of yet). *cough*
I've got to have SOME fun with this... More of a challenge would have been "that LEC who covers the territory once served by C&P Telephone". For folks outside of the region, that may have required some research.
