PDFs/Hyperlinks Nightmare

I've created an InDesign CS 170 page document with an index that contains hyperlinks. When I save as a PDF some of these links go haywire and fail to take the reader to the correct position in the document. Any ideas?
 
Hey, I may be able to but I need a more detailed description. For instance, it sounds like you set up Bookmarks (Internal links). I assume you are creating this for the web? Which Settings are you using?

Sometimes it is a matter of going into the PDF with the text editing tool or the link tool and removing extraneous links that it adds in the PDF process.
